This notice describes how medical information about you may be used and disclosed and how you can obtain access to this information. The Notice of Privacy Practices is made in compliance with the Standards for Privacy of Individually Identifiable Health Information set forth by the U.S. Dept. of Health and Human Services (HHS) pursuant to the Health Insurance Portability and Accountability Act (HIPAA) of 1996, as amended.

Life/AD&D insurance is with Reliance Standard Life Insurance Co. Eligible employees receive a life insurance benefit of two times their salary, rounded up to the next thousand. AD&D (Accidental Death & Dismemberment) has a schedule of benefits for loss of life or limbs due to an accident, up to the life benefit. Beneficiary change forms are available on request from the Benefits Specialist.

Madison National Life Ins. Co is our LTD carrier. Long Term Disability provides a benefit of 60 percent of base wages to be paid after a qualifying period of 60 days or the extent of available sick leave, whichever is greater, upon approval of application. Eligible employees are automatically enrolled in the LTD plan.

Enrollment forms are issued by Human Resources when an employee is hired or becomes eligible for coverage. If a new enrollee has questions regarding the completion of the forms, they should contact the Benefit Specialist. Completed forms should be returned to Benefits or Human Resources.
Enrollment Status Changes
For changes to enrollment status, employees should call or email the Benefits Specialist to inform MPS of the change (birth, death, marriage, divorce, loss of dependent status, change in address or phone number, etc.). Appropriate insurance companies will then be notified by the Benefits Specialist. Changes must be made within 31 days of the date of the event.
Questions regarding the insurances, including claim status, providers, and general coverage inquiries should be directed to the Benefits Specialist.
